Το εστεμμένο αυτό ορειχάλκινο μετάλλιο έχει σχήμα κυκλικό. Στην πρόσθια όψη φέρει έκτυπο ισοσκελή σταυρό πάνω σε διασταυρούμενα ξίφη. Ο σταυρός στο κέντρο φέρει στέμμα και στις κατακόρυφες κεραίες ανάγλυφα τα μονογράμματα των Βασιλέων Γεωργίου Α’ και Κωνσταντίνου Α’. Η κυκλοτερής επιγραφή «ΣΥΝ ΘΕΩ, ΥΠΕΡ ΒΑΣΙΛΕΩΣ ΚΑΙ ΠΑΡΙΔΟΣ 1912-1913» (με βυζαντινότροπη γραφή) περιβάλλει τη σύνθεση. Η οπίσθια όψη φέρει την τρίστοιχη επιγραφή «ΜΑΚΕΔΟΝΙΑ – ΗΠΕΙΡΟΣ – ΑΡΧΙΠΕΛΑΓΟΣ», στεφανωμένη από κλάδους δάφνης. Στην κορυφή της κορώνας υπάρχει κρίκος με οριζόντια μεταλλική υποδοχή από όπου διέρχεται ταινία σε γαλάζιο χρώμα με κόκκινη γραμμή στο μέσον και λευκές στην άκρη. Στην ταινία υπάρχουν οι διεμβολές «ΕΛΛΑΣΩΝ – ΣΑΡΑΝΤΑΠΟΡΟΝ – ΓΙΑΝΝΙΤΣΑ – ΑΕΤΟΡΑΧΗ», σε ανάποδη από την προβλεπόμενη σειρά.

This brass crown medal is round in shape. On the front it bears an embossed square cross, in crossed swords. The cross in the middle has a crown and on the vertical arms of the cross there are embossed monograms of Kings George I and Constantine I. The circular inscription “WITH GOD FOR KING AND COUNTRY 1912-1913” (in Byzantine script) surrounds the composition.. The back side bears the three-line inscription “MACEDONIA - EPIRUS - ACHIPELAGO”, surmounted by laurel branches. At the top of the crown there is a ring with a horizontal metal slot through which passes a light blue ribbon with a red line in the centre and white ones at the edge. On the ribbon there are the words “ELASSON - SARANTAPORON - GIANNITSA - AETORACHI”, in reverse of the intended order.
