Το ορειχάλκινο μετάλλιο έχει σχήμα κυκλικό. Στην πρόσθια όψη φέρει το κεφάλι του Μεγάλου Αλεξάνδρου και κυκλοτερώς την επιγραφή «ΜΑΚΕΔΟΝΙΚΟΣ ΑΓΩΝ 1903-1909». Στην οπίσθια όψη υπάρχει ο αναγεννώμενος φοίνικας με ανοικτά φτερά και κυκλικά η επιγραφή «ΒΑΣΙΛΕΙΟΝ ΤΗΣ ΕΛΛΑΔΟΣ 1936». Η ταινία του μεταλλίου είναι γκρο μεταξωτή ριγωτή, χρώματος κίτρινου στο κέντρο, με δύο πλευρικές μαύρες και κυανές σειρίδες εκατέρωθεν.

The brass medal is round in shape. On one side it has the head of Alexander the Great and a circular inscription “MACEDONIAN STRUGGLE 1903-1909”. On the other side is the phoenix rising with open wings and the circular inscription “KINGDOM OF GREECE 1936”. The medal has a silk striped ribbon, with yellow stripes in the centre, two black stripes on the sides and light blue on either side.
