Πρωτότυπο. Συντομογραφίες αρκετές. Το κείμενο ορθογραφημένο. Στο κάτω μέρος: κυανή μήρινθος, από την οποία κρεμόταν η πατριαρχική σφραγίδα που έχει χαθεί. H περγαμηνή αποτελείται από δύο τεμάχια συγκολλημένα.
